CARER SUPPORT
About the Service:
This funded care may be allocated by DSL for older people and people with other health conditions who need support, and who are living with a person who is their main carer.
Carer Support is an allocated number of funded days per year based on a Needs Assessment via DSL (Disability Support Link). The allocation will be recorded in the individuals WDHB Care Plan.

Carer Support days are to provide a time out for the main carer.

To book Carer Support days, call us or drop in to discuss. We have limited Carer Support available, and care is usually a bed in a shared room.

The Carer Support Form stating current days allocated must be provided to our staff at Admission. We cannot accept an admission without this current Form.
All clients must bring with them personal care items including continence products, medication and a current print out from your GP of medications prescribed, named clothing.

Check in is prior to lunch on the day booked, and discharge is prior to lunch on the discharge date.

Any additional costs incurred for medication supply (scripts), GP visits, continence products or insufficient Carer Support Days will be charged back to the carer.
