About the Service: 

 

Palliative Care/ End of Life patient are admitted through DSL (Disability Support Link). These contracts are for a maximum of six weeks. we provide people with humane and dignified support and services as they face a life limiting condition. After a six week period they will be transferred to a Long-Term Contract
