About the Service: 

 

Previously referred to as R & R, this is a package of care under Residential Respite Services. This care is funded via Waikato or Thames Hospital to support a person on discharge from their facility as a transition back to home after an admission for illness or post operative care.

 

This T2H+ care is usually either 7 or 14 days, based on the patient's needs.

This may be either at Rest Home or Hospital level of care dependent on the patient's level of independence in movement and personal cares.

​

The T2H+ team will assist with this transition and assessment of home needs.

Some patients admitted under ACC may also be supported by the T2H+ team.
